Checkout Options for v4

This add-on adds an ability to include optional or mandatory checkboxes on the last checkout page.

Key features:

• Supports HTML content
• Checkboxes can be mandatory and a customer will have to confirm them before placing an order
• Checkboxes can be arranged in the specific order
• Answers will be displayed on the order details page and in the standard email notifications
